Please follow the instructions below -

1. Open link.
2. Click on 'Click to Sign' in Blue Box.
3. Under VA FMP Enrollment Form enter you
   • a)Fill all the fields in the form and b) Put your mouse-cursor in the 'Sign Here' box and use your cursor to sign.
4. Then after entering your signature click on 'Get Signed Documents'.
